Pre-Application Checklist

Entities wishing to provide healthcare services to migrant and seasonal farmworkers in their area are asked to contact NCFHP's office at (919) 733-2040 to inform of your intent to submit a proposal for funding and to discuss your plan for increasing access to care for farmworkers in your region. NCFHP staff are available to provide guidance and answer any questions you may have.

The process is very competitive and depending upon the availability of funds. In an effort to help potential applicants better understand NCFHP's priority areas and determine the applicant's readiness to apply for funds to serve migrant and seasonal farmworkers in accordance to NCFHP's vision, the following questionnaire is being provided:

We are aware of NCFHP vision, mission, and preferred method for providing services to migrant and seasonal farmworkers.
